Advanced Therapy Aids Stroke Patients
(HealthDay News) -- Therapies that attack blood clots directly in the brain may benefit ischemic stroke patients who don't respond to the standard treatment using clot-busting drugs.
So says a U.S. study that was to have been presented Tuesday at the 20th annual International Symposium on Endovascular Therapy, in Hollywood, Fla.
Ischemic strokes -- which account for about 83 percent of strokes -- occur when a small clot blocks an artery in the brain and halts blood flow, according to the American Stroke Association. If the clot isn't cleared and blood flow restored, the patient will suffer permanent brain damage or death.
"Often patients who fail to improve with standard intravenous (IV) stroke therapy aren't given the chance to succeed with more advanced intra-arterial (IA) therapy, because it's thought that it won't work if IV therapy didn't, and that it will increase the risk of bleeding in the brain," study author Dr. Christopher Zylak, director of neurointerventional radiology at Sacred Heart Medical Center in Spokane, Wash., said in a prepared statement. "Our data suggest that IA therapy can be highly successful even when IV therapy doesn't work, and that the risk of bleeding is no different between the two therapies."
In IV therapy, clot-busting drugs are delivered through an intravenous device in the patient's arm. IV treatment must begin within three hours of the onset of stroke, which means patients must get to the hospital at the first signs of stroke.
IA therapy involves placing a catheter through a small incision in the patient's groin and moving the catheter through an artery all the way to the site of the blockage in the patient's brain. This enables direct delivery of clot-busting drugs to the area. Doctors also can use the catheter to insert a tiny corkscrew-like device to remove the clot.
In this study, the researchers compared 80 patients who received IV therapy and 43 patients who received IA therapy at Sacred Heart from 2004 through 2007. Some of the patients who received IA therapy had not responded to IV therapy.
Success rates of IA therapy (defined as opening up of the blocked blood vessel) were 85.7 percent in 2006 and 83.3 percent in 2007. Death rates among patients who received IA therapy were 30.8 percent in 2006 and 27.8 percent in 2007. That's half the 50 percent to 80 percent death rates published in the "natural history outcomes" of large-vessel strokes, the study authors said.
"Without any question, we definitely were able to help patients who failed IV therapy by providing IA therapy," Zylak said. "In the future, for large-vessel clots, IA therapy may well be the best direct therapy, bypassing IV therapy."
He noted that many stroke victims don't receive any treatment, because they don't recognize the signs (such as vision and speech problems, paralysis, and memory difficulties) and don't seek medical care. The sooner a stroke is treated, the more likely treatment will be successful.
"Overall, we are under-treating stroke. There are many patients who could benefit from stroke treatment who aren't getting it for various reasons," Zylak said. "Treatment therapies today are getting dramatic results. If a medical center doesn't offer the more advanced IA therapy, the patient can be taken by helicopter to a center that performs the therapy, even if IV therapy wasn't successful."
Stroke is the third leading cause of death in the United States, killing about 160,000 people a year, according to the National Stroke Association. About 750,000 people suffer from stroke annually.

FDA Warns Clot Risk Higher in Birth Control Patch Than Pill
(HealthDay News) -- A birth control skin patch used by many American women carries a risk of serious blood clots that is higher than the risk already recognized for the birth control pill, the U.S. Food and Drug Administration warned late Friday.
Based on the results of a new study that discovered the increased danger, the agency said it has approved changes to the labeling for the Ortho Evra Contraceptive Transdermal Patch, to better warn women and their doctors of the potential risk.
"For women that choose to use contraceptives, it is important that they thoroughly discuss with their health-care providers the risks and benefits involved," Dr. Janet Woodcock, deputy commissioner for scientific and medical programs at the FDA, said in a statement.
The warning followed reports out of Canada last week of two deaths, one heart attack and 16 cases of blood clots since 2004 among women who used the patch.
Approved by the FDA in 2001, the patch is sold by prescription and releases ethinyl estradiol (a type of estrogen), and a progestin called norelgestromin via the skin into the bloodstream. But the FDA noted that the body processes hormones delivered in this way differently than hormones delivered in pill form, so that "women using the product will be exposed to about 60 percent more estrogen than if they were using typical birth control pills."
Blood clots called venous thromboembolisms (VTEs) -- which can travel to the lungs as life-threatening pulmonary embolisms -- have long been known to be a rare but potential side effect of the birth control pill, and for the patch as well. In fact, the FDA had already revised the Ortho Evra label once in 2006, after a study found it doubled women's odds for VTE. Another study found use of the birth control patch boosted women's odds for the clots by about the same amount as did the Pill.
However, an epidemiological study just conducted by the Boston Collaborative Drug Surveillance Program, on behalf of drug maker Johnson & Johnson -- has found the patch may carry an even higher clotting risk than the Pill does. According to the FDA, that study looked at women aged 15 to 44 and "found that users of the birth control patch were at higher risk of developing serious blood clots than women using birth control pills."
The patch does have a checkered past. In November 2006, a group of 43 women sued the product's New Jersey-based maker, Ortho-McNeil Pharmaceutical Co. (a subsidiary of Johnson & Johnson), and San Francisco-based distributor McKesson Corp, alleging that they suffered blood clots and other health woes after being placed on Ortho-Evra.
"This product should not be on the market," Shawn Khorrami, an attorney for the plaintiffs, told the Associated Press at the time. "When you put out a product that gives women more hormones than they need, then you're increasing their risk of developing those ailments."
Similar worries have surfaced in Canada. But Janssen-Ortho, the patch's Canadian distributor, defended the product. "The risk of serious adverse events is small in healthy women, but increases significantly if associated with the presence of other risk factors, such as obesity or smoking," the company said in a statement.
The FDA is also stressing proper use of the Ortho Evra patch. In its statement issued Friday, the agency said it believes the product "is a safe and effective method of contraception when used according to the labeling, which recommends that women with concerns or risk factors for serious blood clots talk with their health-care provider about using Ortho Evra versus other contraceptive options."
Sidney Wolfe, director of the Washington, D.C.-based consumer advocacy group Public Citizen, told Toronto's The Globe and Mail newspaper that women "shouldn't use [the patch]. It shouldn't be used because it's a new product with no unique advantage."
One gynecologist begged to differ, however. "When you look at any estrogen-containing contraceptive product available on the market, whether it's the Pill or the Evra patch or NuvaRing, the vaginal ring, all of them slightly increase the risk of blood clot," Dr. Melissa Mirosh, former fellow of the contraceptive advice, research and education fellowship program at Queen's University in Kingston, Ontario, told the Canadian Press.

(HealthDay News) -- Between 5 percent and 20 percent of Americans will get hit by the flu bug this year, but some simple precautions can keep you from becoming one of that number, an expert said.
The flu shot is the most effective method of preventing the flu, said Dr. Robert C. Goldszer, associate chief medical officer at Brigham and Women's Hospital in Boston. It's important to get your shot early in the flu season -- October and November -- and to get a shot every year, Goldszer said. Everyone is eligible for a flu shot, and many places offer it for little or no cost.
If you do start to feel symptoms of the flu (tired, nauseous, congested, achy), get extra rest and drink plenty of fluids -- at least a quart of water every day. Proper hydration and nutrition can help lessen flu symptoms.
If you have the flu, try not to spread it to people around you. Flu viruses are spread by coughing and sneezing. Most people can spread the flu beginning one day before symptoms develop to up to five days after they become ill.
If you do have symptoms, stay home from work or school and avoid public places. Cover your mouth and nose when coughing or sneezing.
Take steps to protect yourself from getting the flu. If you use public transit or work in a crowded place, disinfect your hands regularly and avoid close contact with people who appear to have flu symptoms.

New Test Criteria Spots Rheumatoid Arthritis Sooner
(HealthDay News) -- More patients with early rheumatoid arthritis could be identified and enrolled in clinical studies if the process of diagnosis included one new type of assessment and excluded two traditional assessments, according to a new study.
Researchers looked at 292 people, average age 54, seen at the Arthritis Center of Brigham and Women's Hospital in Boston. The average duration of the patients' symptoms was four years.
Their findings showed that by including anti-CCP (cyclic citrullinated peptide) testing and excluding rheumatoid nodules and radiographic changes, the number of patients correctly classified as having rheumatoid arthritis increased from 51 percent to 74 percent.
When this approach was used in patients who had rheumatoid arthritis symptoms for less than six months (when signs such as nodules and radiographic changes may not yet be apparent), the percentage of patients correctly classified as having rheumatoid arthritis increased from 25 percent to 63 percent.
The study was to be presented Saturday at the American College of Rheumatology (ACR) meeting in Boston.
"Anti-CCP testing is now widely used in clinical practice to aid in the diagnosis of rheumatoid arthritis but is not included in the current ACR criteria for the classification of rheumatoid arthritis. Additionally, rheumatoid arthritis therapies available today are able to slow or halt disease progression. It is important that new therapies are tested early in the disease course before significant damage has occurred," lead investigator Dr. Katherine P. Liao said in a prepared statement.
"The current criteria for rheumatoid arthritis diagnosis include elements that may not become apparent until later in the disease. Minor modifications in these criteria may allow us to correctly identify rheumatoid arthritis patients earlier, when intervention may be more effective," she added.

- Don't stay in one position for too long. Try to move the body's joints and muscles regularly

(HealthDay News) - An annual flu vaccine can help protect you from the nasty effects of the flu virus.

- The injected vaccination contains a killed virus. It is approved for people over the age of 6 months.
- The nasal form of the vaccine is made with a weakened virus, and is approved for people ages 2-49 years. It's not recommended for pregnant women.
- Approximately two weeks after the vaccination, your body develops the antibodies needed to protect itself from specific strains of the flu virus.
- Flu season generally runs from October through May. The best months to be vaccinated are October and November, but you can still be vaccinated later.
- You cannot get the flu from a flu shot, but minor side effects may include swelling, low-grade fever, and aches and pains.
- Similarly, you cannot develop full-blown flu from the nasal spray vaccine, but minor side effects may include runny nose, headache, and sore throat.

A Primer on Cellular Terrain
The term pH, which means "potential hydrogen," represents a scale for the relative acidity or alkalinity of a solution. Acidity is measured as a pH of 0.1 to 6.9 and alkalinity as a pH of 7.1 to 14; a pH of 7.0 is considered neutral. The numbers refer to how many hydrogen atoms are present compared to an ideal or standard solution. Normally, blood is slightly alkaline, at 7.35 to 7.45; urine pH can range from 4.8 to 7.5, although normal is closer to 7.0.Acid-base metabolism refers to the metabolic processes that maintain the balance of acids and bases (alkalines) in body fluids. Acids release hydrogen ions, while bases accept them. The total number of these hydrogen ions present determines the pH of a fluid. Too many hydrogen ions (a pH below 7.0) produce an acidic state called acidosis; too few hydrogen ions (a pH above 7.0) cause an alkaline excess called alkalosis; both can lead to illness.
Oxidation-reduction is the basic chemical mechanism in the cell by which energy is produced from foods. Electrons (negatively charged particles in an atom) are removed from one atom, resulting in "oxidation" of this first atom, and then are added or transferred to another atom, resulting in "reduction" of this second atom. This continual process of energy metabolism is actually a flow of electrons, or a minute electrical current, within the cell.

Other Factors That Can Generate Fatigue

These include:
• Adrenal insufficiency caused by excessive stress and/or the overuse of stimulants (tea, coffee, chocolate, cola, alcohol, tobacco, drugs)
• Anemia (low levels of iron or vitamin B12 in the blood result in anemia; a blood test can verify this diagnosis)
• Candidiasis (often misdiagnosed as CFS in women)
• Cardiovascular causes (if breathless-ness and/or chest pain on exertion accompanies fatigue, the heart may be involved)
• Chronic ill-health (many chronic diseases have fatigue as a symptom)
• Depression
• Diabetes
• Headaches
• Hypoglycemia (low blood sugar)
• Obesity
• Premenstrual syndrome (PMS) (the connection will be obvious if fatigue occurs at the same time in the monthly cycle)
• Sleep disturbance or inadequate
• Stress
A number of the causes listed above are also symptoms of chronic fatigue syndrome. This highlights the circularity of symptoms and illness. Depression is both a symptom and a cause of fatigue, as are headaches, candidiasis, PMS, poor stress-coping skills, and sleep disorders. The presence of these factors can produce a downward spiral of illness and it becomes difficult to tell which came first, the fatigue or the other exhibited symptoms.

CDC Reports More Eye Fungus Infections
U.S. health officials said the number of confirmed cases of a rare eye fungus that can cause scarring of the cornea has climbed to 102. But the source of the infection linked to contact lens cleaners remains unknown, the Associated Press reported.
Bausch & Lomb Inc. stopped U.S. sales of its ReNu with MoistureLoc contact-lens solution on April 10 when the federal Centers for Disease Control and Prevention said it was investigating numerous reports of Fusarium keratitis infections in Americans using the product.
The proportion of patients who said they used MoistureLoc has held steady at around 50 percent to 60 percent of the cases confirmed so far, the CDC said. Other patients have reported using other ReNu brands and six said they used cleaners made by Alcon Inc. and Advanced Medical Optics Inc., the AP reported.
"At this point, it is too early in the investigation to say whether a particular product or solution may be responsible for the outbreak," the CDC said in a statement.

U.S. Buys More Anthrax Vaccine
The U.S. Department of Health and Human Services said it has purchased five million additional doses of Anthrax Vaccine Adsorbed (AVA), a licensed anthrax vaccine, for $120 million.
This supply of AVA anthrax vaccine, from the BioPort Corporation of Lansing, Mich., is in addition to the five million doses of AVA vaccine purchased from BioPort last year. It will be kept in the nation's Strategic National Stockpile, where it will be available in the event of a bioterror anthrax attack, HHS said in a statement released Friday afternoon.
Coupled with an existing supply of antibiotics -- the nation's first line of defense against an anthrax attack -- the additional AVA vaccine should further diversify the nation's stockpile of medical treatments, the statement said.
"We are committed to protecting the nation from the consequences of an anthrax attack," said Stewart Simonson, HHS assistant secretary for public health emergency preparedness.

New Guidelines for Cancer Therapy
The American Society of Clinical Oncology has developed revised guidelines for the use of white blood cell growth factors -- also known as hematopoietic colony stimulating factors, or CSFs -- to prevent a potentially dangerous side effect from cancer treatment called febrile neutropenia.
White blood cell growth factors are proteins that help the body produce the white blood cells that help fight infection. Some cancer treatments can destroy white blood cells, leading to a condition called neutropenia. Febrile neutropenia -- or neutropenia with a fever -- is a dangerous condition that often requires hospitalization.
ASCO's 2006 Update Committee agreed that reduction in febrile neutropenia is an important goal that justified the use of CSFs when the risk of febrile neutropenia is 20 percent or higher, and no other effective chemotherapy regimen with a risk of febrile neutropenia lower than 20 percent is available. The previous guideline had recommended CSF use when the risk of febrile neutropenia was 40 percent or higher, the society said in a prepared statement.
"CSFs are supportive medications, which means they are not intended to treat the cancer, but rather to prevent patients from developing dangerous side effects from cancer treatment," said Dr. Thomas J. Smith, lead author of the guidelines and a medical oncologist with Virginia Commonwealth University's Massey Cancer Center.
The guidelines also include new recommendations for the use of CSFs for patients receiving radiation treatment and for older patients; and updates on recommendations for patients with specific diseases, such as acute leukemia and myelodysplastic syndromes, and for pediatric patients.

FDA Issues Warning on Bowel-Cleansing Products
The U.S. Food and Drug Administration warned Friday of potential kidney failure associated with certain products used to clean the bowel before colonoscopies and other procedures, the Associated Press reported.
Agency officials, in an alert to doctors and patients, said they had documented 22 cases of acute phosphate nephropathy, a rare but serious form of kidney failure linked to the use of oral sodium phosphates. The products cleanse the bowel by causing patients to loose large amounts of fluid through bowel movements, the AP said.
Twenty-one of the cases involved patients given products such as Fleet Phospho-soda or Fleet Accu-Prep. One patient had taken Visicol tablets. None had taken OsmoPrep tablets, a recently approved oral sodium phosphate product, the FDA said.

No Aspartame-Cancer Link, Says Expert Panel
The popular artificial sweetener Aspartame does not increase the risk of cancer, an independent panel of European food-safety experts said Friday.
They reviewed an Italian study released last year that concluded that aspartame was linked to higher rates of lymphoma and leukemia in rats. But the expert panel for the European Food Safety Authority said the number of tumors did not increase in relation to the amount of aspartame given to the rats, the Associated Press reported.
Many of the rats used in the Italian study had suffered from chronic respiratory disease, which was the most likely cause of the tumors, the panel concluded.
"There is no reason ... to undertake any further extensive review of the safety of aspartame," said toxicologist Iona Pratt, who headed the panel.
The findings come a month after the release of a study of half a million Americans that found no link between aspartame and cancer, the AP reported.

California Bill Targets Personal Use of Ultrasound Machines
Call it Cruise Control: The California Assembly voted 55-7 in favor of a bill to restrict the sale of ultrasound machines only to licensed professionals, a move meant to prevent personal use of the medical devices.
The bill, which now moves to the Senate, was introduced after movie star Tom Cruise bought an ultrasound machine to see images of his unborn daughter, who was born last month in Los Angeles, the Associated Press reported.
Doctors criticized Cruise for his actions, noting that improper use of ultrasound can harm a fetus. Technologists and doctors typically receive years of training to conduct ultrasound exams.
The U.S. Food and Drug Administration says laboratory tests have shown that certain diagnostic levels of ultrasound can affect human tissue, the AP reported.
CDC Reports More Eye Fungus Infections
U.S. health officials said the number of confirmed cases of a rare eye fungus that can cause scarring of the cornea has climbed to 102. But the source of the infection linked to contact lens cleaners remains unknown, the Associated Press reported.
Bausch & Lomb Inc. stopped U.S. sales of its ReNu with MoistureLoc contact-lens solution on April 10 when the federal Centers for Disease Control and Prevention said it was investigating numerous reports of Fusarium keratitis infections in Americans using the product.
The proportion of patients who said they used MoistureLoc has held steady at around 50 percent to 60 percent of the cases confirmed so far, the CDC said. Other patients have reported using other ReNu brands and six said they used cleaners made by Alcon Inc. and Advanced Medical Optics Inc., the AP reported.
"At this point, it is too early in the investigation to say whether a particular product or solution may be responsible for the outbreak," the CDC said in a statement.

U.S. Buys More Anthrax Vaccine
The U.S. Department of Health and Human Services said it has purchased five million additional doses of Anthrax Vaccine Adsorbed (AVA), a licensed anthrax vaccine, for $120 million.
This supply of AVA anthrax vaccine, from the BioPort Corporation of Lansing, Mich., is in addition to the five million doses of AVA vaccine purchased from BioPort last year. It will be kept in the nation's Strategic National Stockpile, where it will be available in the event of a bioterror anthrax attack, HHS said in a statement released Friday afternoon.
Coupled with an existing supply of antibiotics -- the nation's first line of defense against an anthrax attack -- the additional AVA vaccine should further diversify the nation's stockpile of medical treatments, the statement said.
"We are committed to protecting the nation from the consequences of an anthrax attack," said Stewart Simonson, HHS assistant secretary for public health emergency preparedness.

New Guidelines for Cancer Therapy
The American Society of Clinical Oncology has developed revised guidelines for the use of white blood cell growth factors -- also known as hematopoietic colony stimulating factors, or CSFs -- to prevent a potentially dangerous side effect from cancer treatment called febrile neutropenia.
White blood cell growth factors are proteins that help the body produce the white blood cells that help fight infection. Some cancer treatments can destroy white blood cells, leading to a condition called neutropenia. Febrile neutropenia -- or neutropenia with a fever -- is a dangerous condition that often requires hospitalization.
ASCO's 2006 Update Committee agreed that reduction in febrile neutropenia is an important goal that justified the use of CSFs when the risk of febrile neutropenia is 20 percent or higher, and no other effective chemotherapy regimen with a risk of febrile neutropenia lower than 20 percent is available. The previous guideline had recommended CSF use when the risk of febrile neutropenia was 40 percent or higher, the society said in a prepared statement.
"CSFs are supportive medications, which means they are not intended to treat the cancer, but rather to prevent patients from developing dangerous side effects from cancer treatment," said Dr. Thomas J. Smith, lead author of the guidelines and a medical oncologist with Virginia Commonwealth University's Massey Cancer Center.
The guidelines also include new recommendations for the use of CSFs for patients receiving radiation treatment and for older patients; and updates on recommendations for patients with specific diseases, such as acute leukemia and myelodysplastic syndromes, and for pediatric patients.

The U.S. Food and Drug Administration warned Friday of potential kidney failure associated with certain products used to clean the bowel before colonoscopies and other procedures, the Associated Press reported.
Agency officials, in an alert to doctors and patients, said they had documented 22 cases of acute phosphate nephropathy, a rare but serious form of kidney failure linked to the use of oral sodium phosphates. The products cleanse the bowel by causing patients to loose large amounts of fluid through bowel movements, the AP said.
Twenty-one of the cases involved patients given products such as Fleet Phospho-soda or Fleet Accu-Prep. One patient had taken Visicol tablets. None had taken OsmoPrep tablets, a recently approved oral sodium phosphate product, the FDA said.

The popular artificial sweetener Aspartame does not increase the risk of cancer, an independent panel of European food-safety experts said Friday.
They reviewed an Italian study released last year that concluded that aspartame was linked to higher rates of lymphoma and leukemia in rats. But the expert panel for the European Food Safety Authority said the number of tumors did not increase in relation to the amount of aspartame given to the rats, the Associated Press reported.
Many of the rats used in the Italian study had suffered from chronic respiratory disease, which was the most likely cause of the tumors, the panel concluded.
"There is no reason ... to undertake any further extensive review of the safety of aspartame," said toxicologist Iona Pratt, who headed the panel.
The findings come a month after the release of a study of half a million Americans that found no link between aspartame and cancer, the AP reported.

Kaiser Permanente Bungled Kidney Patient Transfers: Report
U.S. regulators were overwhelmed and some patients lost out on new kidneys after Kaiser Permanente launched a massive, new kidney-transplant program in 2004, the Los Angeles Times reported.
The HMO started the program without first discussing with regulators how to safely transfer up to 1,500 of Kaiser Permanente's patients to its San Francisco center from two medical centers -- UC San Francisco and UC Davis -- where they were receiving care paid for by Kaiser.
Officials with the United Network for Organ Sharing told the Times that they weren't informed about the need to move hundreds of kidney patients until September 2004, after Kaiser had already opened its new kidney-transplant program. The United Network is the federal contractor that oversees the U.S. transplant system.
The poor planning and paperwork led to long delays in hundreds of patient transfers. Many patients weren't informed that their transfers had not been processed, which effectively put a new kidney out of reach, the Times reported.
Earlier this week, the newspaper reported that 56 transplants had been performed at Kaiser's transplant center, but about twice as many patients on the waiting list had died.
Kaiser has launched an investigation and will not comment until it's complete, a spokesman said.
