Stimulating Thymus Reactivates T-Cell Production

(HealthDay News) -- It's possible to stimulate the thymus gland to produce new immune system T-cells in adults infected with HIV, U.S. researchers say.

HIV infection destroys T-cells, which leads to the collapse of the immune system and severe infection. The thymus gland produces T-cells early in life but gradually loses function and becomes mostly inactive in adulthood. That means it's difficult for HIV-infected adults to produce new T-cells to rebuild their depleted immune systems.

It has long been believed that it wasn't possible to reactivate T-cell production in the thymus. The new study, by researchers at the Gladstone Institute of Virology and Immunology and the University of California, San Francisco (UCSF), is the first to show that therapy can help boost thymus function in adults.

The two-year study of 22 HIV-infected adults found that treatment with growth hormone (GH) increased thymus mass and more than doubled the number of newly made T-cells. The results are published in the March issue of the Journal of Clinical Investigation.

"These results represent new proof-of-principle findings that thymic involution can be reversed in humans," study author Dr. Laura Napolitano, an assistant investigator at Gladstone and an assistant professor of medicine at UCSF, said in a prepared statement.

"Improved T-cell production may be helpful for some medical conditions such as HIV disease or bone marrow transplantation. These findings contribute new information to our understanding of T-cell production and are also an important step to determine whether immune therapies might someday benefit patients who need more T-cells," Napolitano said.

However, much more research is needed to determine whether stimulating production of new T-cells actually provides a health benefit for HIV patients or anyone else, the researchers said.

Portable Computer Devices Can Aid Exercise Programs

(HealthDay News) -- Specially programmed personal digital assistants (PDAs) can help encourage middle-aged and older Americans to increase their physical activity levels, a Stanford University study says.

The 37 study participants were randomly assigned to receive either traditional physical activity information handouts or a PDA loaded with a program that asked questions designed to help users set physical activity goals, track their activity, and get feedback on how well they were meeting their goals.

The device automatically beeped once in the morning and once in the evening to remind users to review the questions. If a person didn't respond to the initial beep, the device beeped three additional times at 30-minute intervals.

After eight weeks, the people with the PDAs had exercised about five hours a week, compared to two hours a week for those with the traditional handouts.

The researchers said they were surprised that PDA users weren't annoyed by the program's additional reminder beeps.

"The PDAs can really keep on you. We were surprised by that; we thought by the time (users) heard the fourth beep, they might find it annoying and not respond at all," Abby King, a professor of health research and policy and of medicine at the Stanford Prevention Center, said in a prepared statement.

In a previous study, King found that automated computer calls were almost as effective as live health educators in encouraging sedentary people to get more active. The researchers feel it's important to find ways to help people boost their physical activity while taking into account their schedules and surroundings.

"Portable computer devices are useful, because they can be carried around throughout a person's day. Such devices represent one kind of strategy for being able to provide individuals with the help and support they need, in a convenient, real-time context," King said.

She noted that cell phones are another example of devices that could help encourage physical activity. "Especially now that we have the iPhone; its big screen would be very useful for providing visual feedback."

The findings were published in the February issue of the American Journal of Preventive Medicine.

Advanced Therapy Aids Stroke Patients

(HealthDay News) -- Therapies that attack blood clots directly in the brain may benefit ischemic stroke patients who don't respond to the standard treatment using clot-busting drugs.

So says a U.S. study that was to have been presented Tuesday at the 20th annual International Symposium on Endovascular Therapy, in Hollywood, Fla.

Ischemic strokes -- which account for about 83 percent of strokes -- occur when a small clot blocks an artery in the brain and halts blood flow, according to the American Stroke Association. If the clot isn't cleared and blood flow restored, the patient will suffer permanent brain damage or death.

"Often patients who fail to improve with standard intravenous (IV) stroke therapy aren't given the chance to succeed with more advanced intra-arterial (IA) therapy, because it's thought that it won't work if IV therapy didn't, and that it will increase the risk of bleeding in the brain," study author Dr. Christopher Zylak, director of neurointerventional radiology at Sacred Heart Medical Center in Spokane, Wash., said in a prepared statement. "Our data suggest that IA therapy can be highly successful even when IV therapy doesn't work, and that the risk of bleeding is no different between the two therapies."

In IV therapy, clot-busting drugs are delivered through an intravenous device in the patient's arm. IV treatment must begin within three hours of the onset of stroke, which means patients must get to the hospital at the first signs of stroke.

IA therapy involves placing a catheter through a small incision in the patient's groin and moving the catheter through an artery all the way to the site of the blockage in the patient's brain. This enables direct delivery of clot-busting drugs to the area. Doctors also can use the catheter to insert a tiny corkscrew-like device to remove the clot.

In this study, the researchers compared 80 patients who received IV therapy and 43 patients who received IA therapy at Sacred Heart from 2004 through 2007. Some of the patients who received IA therapy had not responded to IV therapy.

Success rates of IA therapy (defined as opening up of the blocked blood vessel) were 85.7 percent in 2006 and 83.3 percent in 2007. Death rates among patients who received IA therapy were 30.8 percent in 2006 and 27.8 percent in 2007. That's half the 50 percent to 80 percent death rates published in the "natural history outcomes" of large-vessel strokes, the study authors said.

"Without any question, we definitely were able to help patients who failed IV therapy by providing IA therapy," Zylak said. "In the future, for large-vessel clots, IA therapy may well be the best direct therapy, bypassing IV therapy."

He noted that many stroke victims don't receive any treatment, because they don't recognize the signs (such as vision and speech problems, paralysis, and memory difficulties) and don't seek medical care. The sooner a stroke is treated, the more likely treatment will be successful.

"Overall, we are under-treating stroke. There are many patients who could benefit from stroke treatment who aren't getting it for various reasons," Zylak said. "Treatment therapies today are getting dramatic results. If a medical center doesn't offer the more advanced IA therapy, the patient can be taken by helicopter to a center that performs the therapy, even if IV therapy wasn't successful."

Stroke is the third leading cause of death in the United States, killing about 160,000 people a year, according to the National Stroke Association. About 750,000 people suffer from stroke annually.

Sex Ed Does Delay Teen Sex: CDC

(HealthDay News) -- Sex education programs do work to help discourage many teens from becoming sexually active before age 15, according to data released Wednesday by the U.S. Centers for Disease Control and Prevention (CDC).

Formal programs -- such as those presented in schools and church groups -- did appear to delay onset of sexual activity. For example, teen girls in the nationally representative sample were 59 percent less likely to start having sex before age 15 if they had received sex education, while teen boys were 71 percent less likely, the study found.

"We were obviously hoping to find that sex education is effective. We're glad to see the strong associations," said lead author Trisha Mueller, a CDC epidemiologist. She emphasized that in order to be successful, sex education should take place before young people become sexually active.

Mueller's team also learned that teen boys who attended school were almost three times more likely to use contraception if they had attended a sex education program, compared to those who had not.

However, attendance at a sex education class did not seem to impact girls' use of birth control, the survey found.

The survey did not differentiate between programs that emphasized abstinence and those that educated about contraception. Instead, researchers focused only on whether the teens had ever attended any sex education program in a formal setting, such as school or church.

The study was expected to be published in the January issue of the Journal of Adolescent Health.

According to earlier, 2005 data available from the CDC, 47 percent of high school students said they had already had sex. Of those who were currently involved in a sexual relationship, one-third said they were not using a condom.

Curious about the effectiveness of sexual education on these behaviors, Mueller and colleagues examined data from more than 2,000 teen boys and girls between 15 and 19 years of age who participated in the door-to-door 2002 National Survey of Family Growth.

"Formal sex education is beneficial for youth who are considered to be at-risk," noted Mueller, who cited as an example the 88 percent reduced risk of initiation sex before age 15 among urban black females who had received any sex education. Urban black teen girls who were still in school at the time of the survey had a 91 percent reduced risk of initiation sex before age 15, the survey found.

The research also showed that boys living in single-parent households were more likely to delay sex past age 15 if they had attended a sex education class.

Mueller and her team were interested in teen sexual decision-making before and after the age of 15, because the federal governments' Healthy People 2010 initiative treats 15 as a dividing line. Healthy People 2010 sets a wide array of health goals for states and communities to achieve over the first decade of this century. One of its objectives: to reduce the number of teens under age 15 who are having sex for the first time.

"First and foremost, the report makes clear that the timing of sex education is quite important. That is, providing sex education to young people at an early age seems quite important in helping delay sexual activity," said Bill Albert, deputy director of the Washington, D.C.-based National Campaign to Prevent Teen and Unplanned Pregnancy.

The researchers said the study could not explain why sex education might have a stronger effect in delaying sex among teen boys and black girls, but Albert offered an explanation.

"It is the case that declines in sexual activity among teen boys, as opposed to girls, and African-American teen girls, as opposed to other racial/ethnic groups, have been much more dramatic over the past decade. This may, in part, explain why the effect of sex education seems stronger. It may also be that concern about HIV/AIDs may be particularly strong among these two groups," said Albert.

However, certain sub-populations of teens deserve further research, said Mueller. The data suggested that both rural, white teen girls and white or Hispanic teen girls who had dropped out of school might be more likely to have sex before age 15 if they had sex education, but Mueller said the number of people in those groups in the study was so small that the results could be a statistical fluke.

"They were kind of opposite findings," said Mueller, who acknowledged that "some subgroups may not benefit from sex ed the same way as the larger group of teens."

This research comes in the wake of data released Dec. 5 by the CDC showing that the annual rate of births to teens has increased for the first time in 14 years. Between 2005 and 2006, the birth rate for girls 15 to 19 rose 3 percent -- from 40.5 births per 1,000 in 2005 to 41.9 per 1,000 in 2006.

Considering both studies, Albert said, "The early wins may have been won. Future efforts may well have to be more intense, focused, and creative if the nation is to make continued progress in reducing teen pregnancy and childbearing. Put another way, yesterday's way of doing business will no longer suffice."

Green Tea, Fruit Extracts Touted as Potential Cancer Fighters

(HealthDay News) -- A gel derived from black raspberries, a fruit beverage and old-fashioned green tea all hold potential promise as ways to treat or prevent different types of cancer, preliminary research suggests.

"Until now, foods have not been considered good treatments for chronic illness, especially tackling tumors," Greg Jardine, a biochemist at Dr. Red Nutraceuticals in Australia, said at a teleconference Thursday. In fact, "foods can be medicine," added Jardine, co-author of a manufacturer-funded study of a "punch" that appeared to reduce the growth of prostate cancer in mice.

In addition to Jardine's study, two others -- one with people and one with rats -- suggest that a black raspberry gel can reduce oral cancer lesions and green tea can prevent colorectal cancer.

The studies, which are all small and need further confirmation, were presented at the American Association for Cancer Research's Sixth Annual International Conference on Frontiers in Cancer Prevention Research, which runs through Dec. 8 in Philadelphia.

Researchers at Ohio State University and the University of Kentucky are using a gel made of extracts from black raspberries to treat oral lesions, which often begin as growths inside the mouth and threaten to turn into major tumors.

"About 36 percent will progress to oral squamous cell carcinoma, but, at this point, we don't have the molecular tools to determine which ones will go on to malignancy," said Dr. Susan Mallery, a professor in the Department of Oral Maxillofacial Surgery and Pathology at Ohio State University's College of Dentistry.

About 7,500 people in the United States die each year of oral cancer, according to American Cancer Society statistics, and 34,000 new cases are diagnosed annually.

The researchers assigned 30 patients -- 20 with precancerous lesions and 10 healthy patients -- to apply the gel several times a day. After six weeks of treatment, the grade of the lesions decreased in 35 percent of patients, while it stayed steady in 45 percent of patients and grew in 20 percent, Mallery said.

Patients whose tumors had progressed the least seemed to do the best after undergoing the gel treatment, Mallery said. None of the patients reported side effects.

Why does the gel appear to work? According to Mallery, the active agents appear to be "those chemicals that give it a very rich purple color." The study authors suspect that the chemicals may help "re-educate" tumor cells, reversing their march toward cancer.

In another study, Australian researchers -- funded by Dr. Red Nutraceuticals, the maker of an antioxidant-boosted beverage called "Blueberry Punch" -- gave the drink to mice with tumors that were considered equivalent to prostate cancer in humans.

The tumors were 25 percent smaller in eight mice that drank the punch for two weeks, compared to eight mice that did not drink the punch. Jardine said the next step is to figure out if the punch would have a beneficial effect in men. The punch, designed to be a health aid, is sold in Australia and in New Zealand, and is made of fruit concentrates and a variety of extracts, as well as tarragon, turmeric and ginger, he said.

In the third study, researchers from Rutgers University in New Jersey fed chemicals known as polyphenols from green tea to rats that had colorectal cancer. The polyphenols appeared to reduce the size of tumors by 45 percent, the study authors said.

It's not clear if the polyphenols would have a similar benefit in humans, and the equivalent amount of tea consumption in people would be hefty -- four to six cups a day, the researchers said.

New Test Criteria Spots Rheumatoid Arthritis Sooner

(HealthDay News) -- More patients with early rheumatoid arthritis could be identified and enrolled in clinical studies if the process of diagnosis included one new type of assessment and excluded two traditional assessments, according to a new study.

Researchers looked at 292 people, average age 54, seen at the Arthritis Center of Brigham and Women's Hospital in Boston. The average duration of the patients' symptoms was four years.

Their findings showed that by including anti-CCP (cyclic citrullinated peptide) testing and excluding rheumatoid nodules and radiographic changes, the number of patients correctly classified as having rheumatoid arthritis increased from 51 percent to 74 percent.

When this approach was used in patients who had rheumatoid arthritis symptoms for less than six months (when signs such as nodules and radiographic changes may not yet be apparent), the percentage of patients correctly classified as having rheumatoid arthritis increased from 25 percent to 63 percent.

The study was to be presented Saturday at the American College of Rheumatology (ACR) meeting in Boston.

"Anti-CCP testing is now widely used in clinical practice to aid in the diagnosis of rheumatoid arthritis but is not included in the current ACR criteria for the classification of rheumatoid arthritis. Additionally, rheumatoid arthritis therapies available today are able to slow or halt disease progression. It is important that new therapies are tested early in the disease course before significant damage has occurred," lead investigator Dr. Katherine P. Liao said in a prepared statement.

"The current criteria for rheumatoid arthritis diagnosis include elements that may not become apparent until later in the disease. Minor modifications in these criteria may allow us to correctly identify rheumatoid arthritis patients earlier, when intervention may be more effective," she added.

Study Suggests Cure for Hepatitis C

(HealthDay News) -- Researchers are reporting a potential "cure" for hepatitis C, a blood-borne viral infection that's the leading cause of cirrhosis, liver cancer and the need for liver transplants in the United States.

Use of the drug peginterferon, either alone or in combination with the drug ribavirin, reduced levels of the virus to undetectable levels for up to seven years, the researchers said.

"This paper strongly suggests, for the first time, that hepatitis C is a curable disease," said lead researcher Dr. Mitchell Shiffman, a professor at Virginia Commonwealth University School of Medicine and chief of hepatology and medical director of the school's Liver Transplant Program. "After treatment, 99.6 percent of the patients remained virus undetectable for over five years," he added.

In the study, 997 patients with hepatitis C or with both hepatitis C and HIV were treated with either Pegasys (peginterferon alfa-2a) alone or in tandem with ribavirin. Shiffman's team then monitored blood levels of hepatitis C once a year for an average of 4.1 years, and as long as seven years.

The researchers found that 99 percent of patients with hepatitis C who were treated successfully with peginterferon alone, or in combination with ribavirin, had no detectable virus up to seven years later.

"This is the first long-term study that confirms what we believed for many years that these individuals are truly cured of hepatitis C," Shiffman said.

The remaining eight patients tested positive for hepatitis C at an average of two years after treatment. There was no pattern to the patients as far as age, gender or hepatitis C genotype. It isn't known whether these patients had a relapse or were re-infected with the virus, the researchers noted.

The findings were to be presented Monday at the 38th annual Digestive Disease Week conference, in Washington, D.C.

Hepatitis C is a blood-borne infectious disease of the liver and is one of the most important causes of chronic liver disease in the United States. An estimated 4.1 million Americans have been infected with hepatitis C, and 3.2 million are chronically infected. The number of new infections each year declined from an average of 240,000 in the 1980s to about 26,000 in 2004, the latest year for which statistics are available. The number of hepatitis C-related deaths could increase to 38,000 a year by the year 2010, surpassing annual HIV/AIDS deaths, according to the U.S. Centers for Disease Control and Prevention.

The virus is usually spread through contact with infected blood and blood products. Blood transfusions and the use of shared, unsterilized, or poorly sterilized needles, syringes and injection equipment have been the main routes of transmission in the United States, according to the National Institutes of Health.

Most people who have hepatitis C don't know they have it, Shiffman said. "Of those who have been diagnosed, only about 25 percent have received treatment, because of the side effects of treatment," he said. "The reason why you should treat it is because you can cure hepatitis C, and we finally have the data to definitively document it."

Dr. Eugene Schiff, chief of the division of hepatology and professor of medicine at the University of Miami Miller School of Medicine, agrees that most cases of hepatitis C can be cured.
"In contrast to hepatitis B or HIV, this virus can be totally eradicated and cured," he said.

But, many patients find the side effects of treatment off-putting. Those side effects can include fever and chills, Shiff said. "You feel pretty lousy. After treatment starts, you feel worse the day after your shot, but it tapers off over the course of the week," he said. "Along with that anxiety, irritability and Depression can develop. And we are quick to use antidepressants to allow these people to stay on the medication."

Additional side effects include a drop in the production of white blood cells and anemia. Often patients are giving additional drugs to combat these conditions, Shiff said.

Treatments can go on for as many as 72 weeks, depending on the reaction to therapy Shiff said. "Some people are reluctant to get treatment, because they heard that the treatment isn't so pleasant," he said. "But they should come out and get treatment."

Schiff noted that new antiviral drugs to treat hepatitis C are being tested. "It is hoped that these new antivirals will be more effective and have less severe side effects and may even be used without peginterferon alfa-2a or ribavirin," he said.

Bad Diet? Urine Test May Tell

(HealthDay News) -- A simple test to check potassium levels in urine may help doctors assess and improve patients' eating habits, a Canadian study finds.

Diet plays a key role in overall health, especially when it comes to risks for heart disease, stroke, and cancer risk. But there is no simple, objective and inexpensive way for family doctors to assess a patient's diet, according to the researchers.

Current methods rely on asking patients to report their eating habits on questionnaires or to record their food consumption for several or more days. These approaches are time-consuming, and patients often fail to provide accurate information.

Blood and urine tests provide alternative ways to assess a person's diet. In this study, researchers focused on urinary potassium as a potentially useful marker of a healthy diet.

They noted that foods promoted by current dietary guidelines are good sources of potassium, and evidence suggests that a diet high in potassium reduces the risk of developing a number of health problems.

The researchers collected urine samples from 220 people, ages 18 to 50, who also provided information about their eating habits over the previous year. The participants' blood pressure, heart rate, weight and height were also checked.

The study found a link between increased levels of potassium in the urine, a healthier diet, and lower weight, blood pressure and heart rate.

"These findings suggest, for the first time, that the amount of potassium in the urine is a valid, objective indicator of diet quality," researcher Dr. Andrew Mente, of the Prosserman Center for Health Research in Toronto, said in a prepared statement.

"This urinary marker is a simple, objective, universally available measure of diet quality that may aid physicians in providing effective dietary counseling. Physicians can now establish targets for therapy, monitor the effectiveness of dietary interventions over time, and provide effective dietary counseling to patients at risk because of poor food choices," Mente said.

The study was to be presented Friday at the annual meeting of the American Society of Nephrology, in San Diego.

Hormone Imbalance Could Spur Some Bed-Wetting

(HealthDay News) -- An imbalance in a hormone-like substance called prostaglandin could explain tough-to-treat bed-wetting in some children, Danish researchers report.

Most children have their bed-wetting controlled by a medication called desmopressin, which reduces the amount of urine they produce at night. But about 30 percent of kids don't respond to the drug.

"Our understanding of bed-wetting is continuously improving, and we are getting better in helping children that suffer from the condition." said lead researcher Dr. Konstantinos Kamperis, from the University of Aarhus, Denmark. "How the body treats salt may play an important role in the etiology of the condition."

His team found that children with the type of bed-wetting that does not respond to desmopressin have more salt and urea in their nighttime urine, possibly caused by an imbalance of prostaglandin.

The report is published in the December issue of the American Journal of Physiology-Renal Physiology.

Bed-wetting is a common and bothersome problem. In fact, 5 million to 7 million children in the United States ages six and over wet their beds at night, according to the National Kidney Foundation.

In the study, researchers studied 46 seven-to-14-year-old children suffering from bed-wetting, all of who were treated as outpatients at Aarhus University Hospital. The youngsters had not responded to desmopressin. This group was compared to 15 children of similar age who had no bed-wetting problem.

The children spent two nights at the hospital. The first night was to acclimatize them to the hospital environment. During the second night, the researchers collected blood and urine from the children without waking them.

"We found that bed-wetters excrete larger amounts of salt at night, probably the reason for their bed-wetting," Kamperis said. "Apart from that, these children excrete larger amounts of prostaglandins, and this could explain both the large excretion of salt at night as well as the inability of desmopressin to treat this condition," he explained.

Compared with children who responded to desmopressin, the children who did not respond excreted twice as much urine during the night. In addition, the urine of children who wet their beds during the experiment contained more sodium, urea and prostaglandin than the other children, the researchers found.

"These findings point towards new treatment possibilities for bed-wetting with agents that reduce the amount of salt excreted in urine," Kamperis said. "Such studies are being conducted at the moment. Furthermore, we would be interested in researching the exact etiology of this excess nocturnal salt excretion. That could help our understanding of bed-wetting," he added.
One expert thinks that, while it is possible that prostaglandin might be involved in bed-wetting, the data from this study can't be used to change clinical practice right now.

"This study has some biological plausibility, because some studies suggest that prostaglandin inhibitors are useful in the treatment of bed-wetting," explained Dr. Joseph G. Barone, an associate professor of pediatrics and urology at Robert Wood Johnson Medical School, New Brunswick, N.J. "Prostaglandin inhibitors include Motrin and Advil, but, in my experience, these medications have not been effective against bed-wetting," he added.

Although bed-wetting is very common, there are few basic science studies on this condition, Barone said. "This study adds useful information to the medical literature, and it may lead to further studies. However, clinical recommendations cannot be made based on the results of this study," he said.

Barone noted that desmopressin is not a cure for the problem. "It works in about 50 percent of cases, in my experience," he said. "When desmopressin works, it is not a cure, just a Band-Aid. The theory is that desmopressin reduces the amount of urine at night, and the child does not, therefore, wet the bed."

Bed-wetting continues to be a multifaceted condition that is commonly associated with developmental immaturity, Barone said. "The most compelling evidence that bed-wetting is developmental in nature is the child's natural tendency to outgrow the problem in 99 percent of cases," he said.

Anemone, Shrub Compounds Battle Rheumatoid Arthritis

(HealthDay News) -- Natural compounds from a sea anemone extract and from the rue shrub plant block autoimmune disease responses in both type 1 diabetes and rheumatoid arthritis, U.S. researchers report.

Scientists at the University of California, Irvine, conducted tests on rats and on blood samples from people with type 1 diabetes and on joint fluid from rheumatoid arthritis patients. They found that these compounds worked to deter the effects of destructive T-cells.

Both SL5 (from the sea anemone) and PAP-1 (from the rue shrub) block an ion channel in the T-cells, which prevents these cells from proliferating and producing chemicals called cytokines. These cytokines can attack healthy cells in people with autoimmune diseases.

The findings were published this week in the early online edition of the journal Proceedings of the National Academy of Sciences.

The researchers say it may be possible to use the compounds to develop new autoimmune disease treatments that target the destructive T-cells but still allow other white blood cells to fight disease and infection in the body.

"Autoimmune diseases affect millions of Americans, and any new therapies that can aid them will have great significance," researcher George Chandy of the university's School of Medicine, said in a prepared statement.

"What's promising about this study is that we identified a protein target on the T-cells that promotes autoimmune activity and the compounds that can selectively block the target and shut down the destructive cells," Chandy said.

He and his colleagues are currently conducting preclinical safety studies on PAP-1 and SL5 in collaboration with AIRMID, a San Francisco-area biotech company.
